Important Information

This web page is for the use of debtor(s) and their attorneys who have filed for Chapter 7 and Kenneth A. Welt, is appointed as the Trustee.

Every debtor must attend a First Meeting of Creditors in the United States Federal Courthouse, 299 East Broward Boulevard, Room 411, Fort Lauderdale, Florida (No Cell Phones Are Allowed). When you appear at the meeting, you must bring the following:
  • Government Issued Photo Identification
  • Proof of Social Security Number
  • Insurance Card for Vehicles
(Only one of the following will be accepted)
  • Original SS Card
  • Original Pay Stub
  • Original Medical Card
  • Original W2 Form
NO CELL PHONES ARE ALLOWED IN THE FEDERAL BUILDING.

UPLOAD DOCUMENT INFORMATION


All debtors must supply no later than seven (7) days prior to their meeting the following:

  1. Two (2) months of bank statements. These should be the two immediate preceding months to your month of filing .BRING YOUR BANK STATEMENT FOR MONTH OF FILING WITH YOU
  2. The last federal tax return filed by the debtor (s)


If you were in business for yourself, please upload the following:

1. The two items above plus
2. Copies of accounting data electronically ( Quikbooks, etc.)
3. Go to and complete form on "Business Form" Tab.

In Addition:

You might receive notification from Trustee's Office that additional for,ms must be submitted. Please note that this request will be sent separately.

You must bring with you VIN numbers for all vehicles (on insurance cards) as well as copy of all bank statements for the month of filing.

1 To access DocLinkTM, please email the trustee to request a unique Registration Key.

2 Connect to the DocLinkTM site via the e-mail link or at https://doclink.bms7.com.

3 Create an account using the Registration Key provided by the trustee, and establish a unique User Name and Password. For multiple trustees, simply add each Registration Key to your existing account.
